Postmortems
We will all fail; that's not a problem. The problem is if we fail the same way twice. Postmortems are retrospectives in which we record
a timeline of what went wrong, how it was discovered, and steps taken to address it
an analysis of why it went wrong, and
propose practices or systems to prevent it from happening again.
