We will all fail; that's not a problem. The problem is if we fail the same way twice. Postmortems are retrospectives in which we record
- a timeline of what went wrong, how it was discovered, and steps taken to address it
- an analysis of why it went wrong, and
- propose practices or systems to prevent it from happening again.
